Kaiser Rehabilitation Center’s social worker communicates information on a patient’s progress to the patient and the family. He or she assists the patient and family in understanding the disability and identifying economic, equipment, care-giving, and social resources prior to discharge.
Social workers are able to perform a psychosocial assessment and assist in discharge planning, family training, and community reintegration guidance for patients and their families.
