Center for Diabetes Management

Diabetes Education makes a Difference.

Recognizing that a one-size-fits-all approach doesn't work for diabetes management, the Center's team of highly trained dieticians, nurses, and specialists work with each patient to tailor a program that includes diet, exercise, and medication to fit his or her lifestyle. Group sessions and one-on-one support allows patients to learn the skills they need to take control of their diabetes and live healthier lives.

7 vital steps to successful Diabetes self care:

  • Healthy Eating
  • Being Active
  • Monitoring
  • Taking Medications
  • Problem Solving
  • Healthy Coping
  • Reducing Risks

2 simple steps to schedule an appointment or a class

  1. A physician referral is required. Ask your physician to fax or mail a referral to the Center.
  2. After we receive your physician’s referral our staff will confirm your insurance benefits (Many insurance plans and Medicare cover these services for their members), then call you to schedule an appointment.
