PAD Rehab

At Hillcrest Exercise & Lifestyle Programs (HELP) we offer a multidisciplinary rehabilitation program for patients with peripheral arterial disease. To participate in the program, patients must be referred by their physician. Once referred, the program option will be determined by the patient's symptoms and insurance coverage. The recommended length of time to maximize the patient's health benefit is 6 months.

Exercise:

Option 1 - Patients that require monitored supervision will participate up to the limit of insurance coverage, subject to any applicable co-pays or deductibles. This phase of the program generally consists of three sessions per week for 12 weeks, but may vary depending on coverage. Exercise sessions will be supervised by a Cardiac Nurse Specialist. Once this portion of the program is complete, the patient will transition to option 2 (below) which requires no monitoring. This phase of the program will be supervised by an Exercise Physiologist.

Option 2 - Patients referred with less severe symptoms, will receive two monitored sessions at the start of the program. These sessions will be supervised by a Cardiac Nurse Specialist. Once the monitored sessions are completed, the patient will continue the program without monitoring at a cost of $100 per month under the supervision of an Exercise Physiologist. Class sessions will be three times each week Monday, Wednesday and Friday at 2pm. The cost of this self-pay portion will be $25 weekly or $100 per month.

Patient Education:

Classroom education for PAD rehabilitation patients consists of four classes. Topics covered include cardiac anatomy and physiology, treatment options for heart disease, risk factors of heart disease, heart medications, stress management, heart healthy nutrition and exercise. Participants will receive written information highlighting what is covered in each of these classes. There is no charge to attend these classes.

Smoking Cessation:

This Smoke Free course provides assessment and education for participants who want to quit smoking. A Behavioral Health Specialist will guide each patient through a sound behavior and attitude modification process towards achieve smoking cessation goals for a lifetime. Four one-hour sessions provide the necessary information and strategies for success. It is recommended that 10 to 12 days in advance of the first session, patients contact their personal care physicians about their intention to quit smoking and request education, if appropriate, to assist the quitting process. The cost of the program is $220.
